Welcome to Klarity Mobile Phlebotomy. These Terms & Conditions govern the use of our mobile phlebotomy services. By scheduling an appointment with Klarity Mobile Phlebotomy, you agree to abide by these terms.

Services Provided

Klarity Mobile Phlebotomy offers in-home and on-site blood draw services. We work with licensed and trained phlebotomists to ensure safe and efficient sample collection.

Scheduling & Appointments

​

  • Appointments must be scheduled in advance.

  • A valid government-issued ID must be presented at the time of service.

  • Cancellations or rescheduling require at least 24 hours' notice. Late cancellations may incur a $25 fee.

Payment & Fees

​

  • Payment is due at the time of booking unless prior arrangements have been made.

  • We accept major credit/debit cards and other specified payment methods.

  • Additional fees may apply for emergency or after-hours services.

Client Responsibilities

​

  • Clients must provide accurate information regarding their identity and health history.

  • Clients should be properly hydrated and follow any pre-test instructions given.

  • A responsible adult must be present for minors or individuals requiring assistance.

Liability & Disclaimer

  • Klarity Mobile Phlebotomy is not responsible for any adverse reactions or complications arising from blood draws.

  • We do not provide medical diagnosis or treatment recommendations.

  • In case of medical emergencies, clients should seek immediate medical attention.

Privacy & Confidentiality

  • We comply with HIPAA regulations to protect client information.

  • Personal and medical data collected will only be used for service purposes.

  • No client information will be shared with third parties without consent.

Consent & Authorization

By using our services, you consent to the collection and handling of your blood samples. You authorize us to share your results with the ordering healthcare provider.

 

Amendments

We reserve the right to update or modify these terms at any time. Continued use of our services constitutes acceptance of any changes.
