Klarity Mobile Phlebotomy operates as an independent "fee-for-service" platform. We do not submit claims to insurance for our services. Instead, a one-time "fee for service" will be charged directly for each scheduled visit. Currently, we accept payments via Credit Card and Debit Card.

It is important that your insurance details are included on the physician's requisition form to ensure that the designated laboratory can process your specimens. Please verify that the requisition is completed accurately before uploading any documents.
